Issues can be reported anonymously for any participant, at any time. Go to www.convercent.com/report. This will bring you to the 'Report an incident' page. From this page the reporter can access their organization’s landing page to file a report. Test it by typing your organization's name in the field as shown below.
Your organizations landing page will look similar to this. From here you will be able to file the report. You will also be able to change the language to make navigation easier if necessary.
Start the process by selecting both the location of the incident, and your location as the reporting party. Click “Get Started” to continue.
You will next be prompted with a notification that this isn’t an emergency service. If your issue is an emergency please contact your local police or emergency responders immediately. After acknowledging that you understand this, click “Get started with your report” to continue.
**NOTE: If your organization uses a simple intake, you will skip these next two steps.**
If your organization uses a guided intake, you will see this screen. Select the category of your report from the options provided.
A dropdown menu will appear with a series of questions for you to answer after you select your issue type. Answer the questions as accurately as you can, then click “Tell us what happened” to continue.
In the “Tell Us What Happened” section you will be asked to fill out certain fields. The first of which is a description of what occurred. In here you are encouraged to provide as many details as you can. Secondarily, you will be asked to add involved parties to the issue. Whether that is the subject, witnesses, or other victims.
You will next be asked to provide the issue date and location. You can select a specific department, as well as the location from a selection menu. If you can’t find your location, you can enter a street address to provide information. You will be asked to provide a specific date, or an approximate one if you aren’t sure.
